Heart failure (HF) is a common pathology that affects over 6 million adults. The lifetime risk of HF is 1:9 for males and 1:6 for females, and increases greatly in individuals who have experienced prior myocardial infarction (MI). Thus, treatment and prevention of HF are a major priority for most health care professionals, yet poor prevention and care are the most cited reasons for adverse clinical outcomes.

In 2014, the FDA approved the drug Entresto for the management and treatment of HF, finding that it significantly reduced the risk of death from HF-related conditions by 16-20% and was particularly effective at reducing the risk of death for HF patients with reduced left ventricular ejection fraction (LVEF). Entresto is actually a combination of two drugs: sacubitril, which inhibits the enzyme neprilysin, and valsartan, which is an angiotensin II receptor antagonist.

How do these drugs work together to reduce the risk of further heart failure and preserve function? In your response, briefly describe the two signaling pathways that are affected by the drug, how they are affected, and what the body’s overall response to the drug intervention is. How does this benefit patients with HF?

Before responding to this discussion, please read the Scientific Method Tutorial and if needed review optional readings.

Describe an example of how you use the scientific method in your daily life. Which observation(s) lead you to use the scientific method? State a good hypothesis, how you are testing the hypothesis (the experiment), results, and conclusion. What are the control and treatment groups in your experiment? What are the dependent and independent variables in your experiment? Name at least one controlled variable.

Your main response should be a minimum of 150 words (more is OK). You must also post a minimum of one significant reply to a classmate’s post that contributes to their learning. The reply should be a minimum of 100 words.

Scenario

You work as a middle manager for one of the top U.S. producers of luxury and mass-market automobiles and trucks.

The chief technology officer (CTO) of the company from the course scenario has been watching new technology developments that the company could integrate into its vehicles to enhance the usefulness of and access to the data acquired by the many digital sensors that have been integrated into vehicle subsystems over the past 20–30 years. The technology trend of particular interest is the internet of things (IoT)—the interconnection of embedded devices, such as sensors and computers, over the internet. By taking advantage of this trend, the CTO believes the company can seize an opportunity to provide better service and predictive maintenance to its customers, improving customer satisfaction and adding additional revenue streams.

Based on briefings by the CTO, senior management has decided to implement IoT in its product line. Your CTO has asked you to lead a cross-functional team to take this initiative forward. Your first task is to make a recommendation for how the company should approach this business problem. Do you recommend using incremental or discontinuous (that is, slow or fast) innovation? Specifically:

Now, the CTO has asked you to come up with a proposed organizational structure that will support the innovation implementation. In this way, you will be assisting in making the innovation sustainable for the company from the course scenario. Companies that are innovative must make sure they have structured the organization in a way that supports innovation, and a key component of innovation is keeping track of what their customers want in terms of products and services.

You are a DNP-prepared nurse working at a hospital focused on improving patient satisfaction. After receiving care at your hospital, patients are provided a scorecard to survey their patient experience. The patient surveys range in questions from wait time to effectiveness of care, and these surveys provide your hospital with a scorecard indicating how the hospital is performing against these metrics. Upon reviewing the scorecards, you are able to highlight areas of improvement and areas of success, however, you find the responses are often difficult to analyze, as there are a wide range of responses, and there are many variables.
The process of constructing a balanced scorecard for the tracking of patient satisfaction can be controversial. For example, a hospital’s patient satisfaction scorecard provides a snapshot of gathered data for the hospital, but the data may be out of context, which makes it difficult to identify specific problems.
