  2. Demonstrate professional development related to contracted learning experiences and the traditional-year competencies
  3. Reflect on professional growth and development
  4. Provide information that facilitates dialogue between the student and Field Supervisor
  5. Document for the Field Supervisor, concerns, dilemmas and issues as they arise in the agency setting.

After the passage of the Patient Protection and Affordable Care Act of 2010, health care organizations have been faced with significant challenges in providing quality care to all Americans. HealthyPeople.gov  (https://health.gov/healthypeople) also encourages health care organizations to focus on the relevance of social determinants and health status.                                                                                                                                                

Take on the role of the administrator of a community hospital in Atlanta, Georgia. You would like to implement a strategic plan to improve the health status of your community. Select a vulnerable population in Atlanta, Georgia affected by a disease or condition. Examples include aging, COVID-19, diabetes, Ebola, heart disease, opioid epidemics, Zikavirus, etc. In three to five pages, detail your strategic plan to address the following:

  • Describe the population, including demographics and risk factors that determine health in this population.
  • Explain the disease or condition prevalent in this population.
  • Identify access and barriers to health care and treatment options for this population, including local, state, and federal policies regulating the control and prevention of your selected disease or condition in this population.
  • Propose at least three strategies to improve the health of the selected population.
  • Develop at least three key indicators to measure the success of your proposed population health management.

For the third milestone of your final project, submit an overview of the communication tool and media campaign plan. Identify relevant audiences for your media campaign and how you plan to interact with them. Provide the critical information that you need to disseminate to the population at risk (primary audience). Identify other relevant audiences (secondary or tertiary) and discuss how information prepared for them may differ.

Discuss considerations for presenting appropriate information to the relevant audiences including elements of health literacy and numeracy (e.g., communication skills, knowledge, culture and English language proficiency specific, representative heuristic). Review research on the theories used to predict persuasion and guide behavior change that might be appropriate for communicating this critical information to your target audience(s) and describe how you will incorporate these in your communication strategy.

Discuss the communication tool that will be the most effective for disseminating the information that will be used in your media campaign (e.g., social media, news article, report card, PSA). Explain why this tool will be an effective means of communication for the chosen health topic with the targeted population.

Discuss legal and ethical standards and policies that must be considered in developing the media campaign and how these apply to your population at risk. Propose potential partners in your campaign and how you might involve them.

How do social workers apply a strengths-based and person-in-environment perspective to different populations and social problems? First, social workers assess the identified client and the client’s environment, recognizing that the interaction between person and environment may be creating or contributing to the problem. Depending on the assessment, the target for change may be the person and/or the environment. Second, social workers assess client strengths and then utilize those strengths during the change process. A strengths-based perspective asserts that all individuals, families, groups, and communities have strengths.

As a BSW social worker, you may be placed in the role of case manager, and this week’s assignment will help you consider that role when working with a specific population.

Discussion: Interventions for Survivors of Sexual Trauma

One in 6 females and one in 33 males have experienced some form of sexual trauma in their lives (RAINN, 2009). Many who seek treatment present with issues unrelated to sexual trauma. Given the high percentage of sexual trauma survivors, it is very likely that you will encounter clients with such a history during your career, regardless of your specialization. Therefore, it is important to familiarize yourself with a sexual trauma framework and skills specific to working with this population. While sexual trauma mostly occurs on an individual level, it also can have a significant impact on the family system. As a practitioner, you may intervene with the individual survivor, the parent(s) of a child who has been sexually abused, the spouse of a rape survivor, and/or the family system as a whole.

For this Discussion, select two types of sexual trauma from the readings this week and think about two interventions you might use for each and why.
