HS111: Medical Terminology

CLASSMATE POST #1

A complete documentation in a patient’s medical record can
greatly improve their continued care. For example, having the correct symptoms
notated could allow a different healthcare professional to identify the cause
of an illness. Knowing a patient’s history not only helps the patient
feel heard and understood but it allows the healthcare team to provide accurate
care.

An incomplete documentation in a medical record can possibly
have deadly results but definitely negative ones. For example, the wrong
medication could be given, and the patient could have a severe reaction to it.
Knowing correct terminology and directions of the body is important when
documenting in a medical record because it helps eliminate mistakes.

Complete and accurate documentation means a lot for the future
care of patients because it can encourage them to speak out and come to a
hospital or clinic for help. Accuracy also gives healthcare professionals
reliable information so they can cure/help their patients. Unfortunately,
incomplete documentation in medical records can lead to symptoms and illnesses
being misdiagnosed or just missed all together. Inaccurate documentation
discourages patients from getting help and takes away the trust between them and
the healthcare team.

Having a
complete documentation in a medical record is very important because patients
go to different health care settings such as doctors’ offices, hospitals,
urgent cares, and more. Having complete documentation in your medical record
helps doctors from all over to take care of you in the best way possible by
knowing your past medical history, allergies and more.

Complete documentation should the process of collecting
subjective history, gathering objective data, formulating an assessment, and
developing an action plan. This pattern is presented as SOAP, an acronym that
stands for Subjective, Objective, Assessment, and Plan. (Jones, S. L, 2019,
pg.56)

Incomplete documentation could reflect poor patient care.
Important information could be missed such as a patient’s allergies, tests that
were done, important symptoms, and more. That’s why the SOAP method is very
important.

A good example would be a daily hospital note, or progress note.
This helps everyone in the hospital stay informed including nurses and the
patients. This would include a subjective, objective, and the assessment and
plan are used together. It would explain any changes since the previous note
with the assessment and then explaining what the plan is for the day. If this
documentation was incomplete, then the hospital staff and the patient would be
confused as to what is going on and what the plan is.

CLASSMATE POST #3

Human Papillomavirus is a STD that is relatively mild when it comes to symptoms, but can have some serious complications. The main symptom of HPV is warts. Most immune system’s will kill the cells before warts appear. However, if they do appear, they can present on your hands, face, or genitals. No fun!

The main concern for HPV is cervical cancer. Nearly all cases of cervical cancer result from a HPV infection. Since there are little to no symptoms, doctors recommend routine Pap tests.

The CDC recommends two doses for the vaccine. The first dose comes around 11-12 years of age, and the second dose should come 6-12 months later. It’s not recommended to have a vaccine after the age of 26, but doctors may suggest it due to certain lifestyles.

A person who is exposed with latent tuberculosis are not infectious and cannot spread TB to others. Tuberculosis is a disease caused by a germ called Myobacterium tuberculosis that is spread from person to person through the air. Tuberculosis affects the lungs and becomes a droplet isolation.

Tuberculosis in pregnant women is very dangerous, especially since the infant can contract TB. Latent TB infection requires Isoniazid daily or twice weekly for nine months with pyridoxine(vitamin B6) supplement. Making sure the infant and mother are safe during pregnancy is important no matter what the circumstances. There are a lot of things that can happen with pregnant women given their inability to take a lot of different medications.
